If a donation was made to a hospice can another family member with the same last name, claim the donation on their income tax?

    Only the person that made the donation can take the deduction.

    No.  You cannot claim anything.  It's gone, in the other person's name.  First and Last name, probably with details.

    No.  All of the information would need to match.

    Also - charity donations don't mean anything unless you have enough deductions to itemize your deductions.  Charity donations are part of the itemized deductions.

    You can have either the standard deduction for your filing status or you can itemize your deductions.  You can't use both.
